Life sometimes presents with challenges and obstacles that seem impossible to get through. One may often times feel alone and that there's nowhere to turn; but there truly is. I look at the whole individual and their experience as unique, and focus on helping the person reveal their true self: the person they always wanted to be.

INSURANCE
IN NETWORK Regence BlueShield Kaiser PPO & HMO Premera First Choice Health Blue Cross Blue Shield LifeWise Please bring your insurance card to your first session. You may need to contact your insurance company for pre-approval before you can start counseling. To better understand your benefits, please contact your insurance providers customer service line for an explanation of your benefits. I contract with a medical biller who will submit your claims on a weekly basis. OUT-OF-NETWORK Most insurance plans have mental health benefits for out-of-network providers. I can work with you to understand what your insurance benefits cover. You will pay me up front for session. I then provide you a receipt at the beginning of the session for you to submit to your insurance company for reimbursement. I can provide a clinical intake/mental health evaluation for individuals who have experienced an injury due to an auto accident. Please have claim information available. |
FEES
FIRST SESSION First sessions are 55-60 minutes in duration that includes signing forms for Authorization to Treat, counselors Disclosure Statement that includes info on office policies and client rights and the New Patient Form. Additionally, I will conduct a clinical interview to get a better understanding of which direction the counseling sessions should head therapeutically. For the following forms please see the website section "Forms and Resources" If you wish to use your insurance, please review the list of insurance companies and plans I accept and bring your insurance card with you to your first session. Individuals that are out of network or no insurance available can discuss payment plans and pricing at the time of service. Cash, check, credit card, HSA and FSA cards are accepted forms of payment. |
CANCELLATION AND MISSED APPOINTMENT POLICY
I require 24 hours notice if you need to cancel or reschedule your counseling appointment. For further info on my cancellation policy and past due balances please see my disclosure statement. Cancellations with less than 24 hours notice-$50 Missed appointments-$75 |
